ORDER of the Ministry of Health and Social Development of the Russian Federation of 23.07.2010 N 541n "ON APPROVAL OF THE UNIFIED QUALIFICATION REFERENCE BOOK OF THE OFFICES OF MANAGERS, SPECIALISTS AND EMPLOYEES, SECTION" QUALIFICATION CHARACTERISTICS OF WORKERS
Section "Qualification characteristics of positions of workers in the health care sector" I. General provisions1. Section "Qualification characteristics of positions of workers in the field of health care" of the United qualification handbook positions of managers, specialists and employees (hereinafter - TSA) is designed to promote the correct selection and placement of personnel, improve their business qualifications, division of labor between managers, specialists, as well as ensure the unity of approaches in determining job duties these categories of workers and the qualification requirements imposed on them and consists of five sections: I - "General provisions"; II - "Positions of managers"; III - "Positions of specialists"; IV - "Positions of junior medical personnel"; V - "Positions of other personnel in medical and labor workshops at medical organizations."
2. The qualification characteristics contained in the section "Qualification characteristics of health care workers" of the CEN (hereinafter - qualification characteristics) are used as regulatory documents, and also serve as the basis for the development job descriptions containing a specific list of job duties, taking into account the peculiarities of the work of workers in medical organizations. The job titles of specialists with higher medical and pharmaceutical education are established in accordance with the Qualification Requirements for specialists with higher and postgraduate medical and pharmaceutical education in the field of healthcare, approved in accordance with the established procedure by the Ministry of Health and Social Development of Russia. For example, the job title "Physician" corresponds to the medical specialty "Therapy".
The names of the positions of medical specialists and specialist pharmacists are established in accordance with the names of medical and pharmacist specialties, the nomenclature of which is approved in the prescribed manner.
The title of the position "doctor-laboratory assistant" is retained for specialists with higher professional (non-medical) education, hired for this position until October 1, 1999.
In the case when a specialist doctor (pharmacist) has a break in work in a specialty for more than 5 years or changes the profile of a specialty that requires special training and qualifications, he is appointed to the position of a trainee doctor (trainee pharmacist) for the period of professional retraining in accordance with the established procedure with the aim of obtaining special knowledge, skills and abilities necessary for independent work in the position of a specialist doctor (pharmacist), and obtaining a certificate in the relevant specialty.
During this period of time, he can perform the duties provided for by the qualification characteristics of a specialist doctor (doctor - specialist of the state sanitary and epidemiological service institution, pharmacist), only under the guidance of a specialist doctor (doctor - specialist of the state sanitary and epidemiological service institution, pharmacist) who has a certificate of a specialist in the relevant specialty, answering on a par with him for his actions.
3. The titles of the positions of the deputy heads of a medical organization (chief physician, director, head, chief) shall be supplemented by the name of the section of the work, which he manages. For example, "deputy chief physician for medical care", "deputy chief physician for work with nursing personnel", etc.
The titles of the positions of the heads of structural divisions (departments, departments, laboratories, offices, detachments, etc.) are supplemented by the name of a medical position corresponding to the specialty in the profile of the structural division. For example, "the head of the surgical department is a surgeon."
4. The qualification characteristics of each position contains three sections: "Job responsibilities", "Must know" and "Qualification requirements".
In the section "Job responsibilities" there is a list of basic functions that can be assigned to an employee holding this position, taking into account the technological homogeneity and interconnectedness of the work received vocational education.
The section "Should know" contains the basic requirements for an employee in relation to special knowledge, as well as knowledge of legislative and other regulatory legal acts, regulations, instructions and other documents, methods and means that the employee must be able to use in the performance of job duties.
The section "Requirements for qualifications" defines the levels of the required professional education of the employee, necessary to perform the assigned job duties, as well as the required length of service. The levels of required vocational training specified in the "Qualification Requirements" section are given in accordance with the Law Russian Federation"On Education" dated July 10, 1992 N 3266-1 (Collected Legislation of the Russian Federation, 1996, N 3, Art. 150) and other regulatory legal acts.
5. The official title "senior" is established on condition that the specialist is in charge of subordinate performers.
6. Persons who do not have appropriate additional professional education or work experience, established by qualification requirements, but have sufficient practical experience and performing the job duties assigned to them with high quality and in full, on the recommendation of the certification commission of the medical organization, as an exception, can be appointed to the appropriate positions as well as persons with special training and the necessary work experience.
The employer shall take measures for the further professional training of the said persons.
7. In medical organizations, job titles: "midwife", "nurse", "nurse", "packer", replaced by males, are named respectively: "midwife", "medical brother (nurse)", "orderly", " packer ".
II. Positions of managers Chief physician (president, director, head, manager, head) of a medical organization<*>The title of the position "Chief Physician" can be used only if the head of a medical organization has a higher professional (medical) education.
Job responsibilities. Supervises the activities of a medical organization. Organizes the work of the organization's team to provide and provide quality medical services to the population. Provides the organization of treatment-and-prophylactic, administrative and economic financial activities organizations. Analyzes the activities of the organization and, based on the assessment of its performance indicators, takes the necessary measures to improve the forms and methods of work. Approves staffing table, financial plan, annual report and annual balance sheet of the organization. Ensures the fulfillment of obligations under the collective agreement. Provides employees with timely and full payment of wages. Improves the organizational and management structure, planning and forecasting of activities, forms and methods of work of the organization, carries out the selection of personnel, their placement and use in accordance with qualifications. Organizes and ensures that employees of the organization receive additional professional education (advanced training, professional retraining) in accordance with the legislation of the Russian Federation. Takes measures to ensure that employees of the organization perform their duties. Ensures and monitors the implementation of the internal labor regulations, labor protection and fire safety during the operation of devices, equipment and mechanisms. Represents the organization in state, judicial, insurance and arbitration bodies, at international events, in state and public organizations on issues in the field of healthcare, previously agreed with the higher authority in charge of this organization. Interacts with local authorities, civil defense services, disaster medicine, territorial internal affairs bodies and other operational services. Takes part in conferences, seminars, exhibitions. Ensures the implementation of preventive measures to prevent industrial injuries and occupational diseases.
Job responsibilities. It dispenses sanitation and hygiene items, nursing care, other medical products, and medicinal herbs. Provides storage and control over the shelf life of medicinal herbs and medical products. Participates in the acceptance, placement and packaging of goods, using the necessary equipment and observing the rules of safety and labor protection. Conducts sanitary, educational and informational work on the use of medical devices.
Should know: the basics of the organization of the pharmaceutical service; basic regulatory legal acts on pharmacy issues; nomenclature of medicinal herbs and medical devices; fundamentals of labor legislation; internal labor regulations; rules for labor protection and fire safety.
Qualification requirements. Secondary vocational education in the specialty "Pharmacy" without presenting requirements for work experience.
Sister-hostessJob responsibilities. Supervises the work of nurses and cleaners to keep the premises of the medical organization (unit) clean and tidy, provides the serviced unit with household inventory, overalls, hygiene items, stationery, detergents, bedding and underwear for patients. Changes gowns and towels for medical workers. Draws up applications for the repair of premises, equipment, inventory and controls its implementation. Provides power supplies (buffet, canteen) with equipment, utensils and monitors their correct labeling and use. Keeps accounting and reporting documentation.
Should know: expiration dates of linen and equipment used in a medical organization (department); methods of sanitizing inventory; operating and storage conditions for inventory; forms of accounting and reporting documentation and the rules for filling them out; rules for compliance with the sanitary and hygienic regime in a medical organization (unit); internal labor regulations; rules for labor protection and fire safety.
PackerJob responsibilities. Carries out the work on the packaging and dosage of medicines, medical devices, using the necessary equipment, mechanization means, observing the rules of their operation. Participates in the acceptance and distribution of goods between the departments of the pharmacy.
Should know: basic methodological and regulatory documents on pharmacy issues; internal labor regulations; rules for labor protection and fire safety.
Qualification requirements. Secondary (complete) general education and additional training in the direction professional activity without presentation of requirements for work experience.
NurseJob responsibilities. Cleans premises in a medical organization. Helps the eldest nurse upon receipt of medicines, instruments, equipment and their delivery to the department. Receives from the hostess sister and ensures the correct storage and use of linen, household equipment, dishes and detergents. Cleans bedside tables from bedridden patients after each meal. At the direction of the ward nurse, he accompanies patients to diagnostic and treatment rooms. Performs the functions of a courier, carries out washing of pharmaceutical dishes. Informs the hostess sister about malfunctions in the heating system, water supply, sewerage system and electrical appliances. Prepares rooms and baths. Systematically (after each patient) carries out sanitary and hygienic treatment of the bathtub and washcloths. Provides assistance to patients while taking a hygienic bath, while undressing and dressing. In the absence of a junior nursing nurse, he receives underwear and bed linen from the hostess sister and makes a change. Receives ready-made food at the catering unit, checks it by weight and count. Signs in the handout. Heats food. Distributes hot food to patients according to the menu and the prescribed diet. Performs dishwashing, cleaning of the pantry and canteen, observing sanitary requirements... Systematically performs cleaning of refrigerators intended for storing patients' food. Provides sanitary and hygienic maintenance of the pantry and canteen. Timely informs the management of the department about the need to repair the equipment and inventory of the pantry.
Should know: rules of sanitation and hygiene at work; the purpose of detergents and the rules for handling them; internal labor regulations; rules for labor protection and fire safety.
Qualification requirements. Secondary (complete) general education without presentation of requirements for work experience.
Orderly driverJob responsibilities. Together with the paramedic, she carries out the transportation, loading and unloading of sick and injured persons during their transportation, assists the doctor and paramedic in performing diagnostic and therapeutic procedures, and transfers medical equipment. Provides assistance to medical personnel accompanied by patients. Drives an ambulance that is categorized as "B" or "C". Fills the vehicle with fuel, lubricants and coolant. Draws up travel documents. Checks the technical condition and acceptance of the car before going on the line. Delivery of the car and staging it in the designated place upon returning from work. Eliminates minor operational malfunctions of the rolling stock that occurred during operation on the line, which do not require disassembly of mechanisms. Carries out sanitary and hygienic treatment of the car interior after each patient (victim). Informs about vehicle malfunctions.
Should know: rules of operation of the used medical instruments and equipment; safety rules when working with medical instruments and equipment; psychology of professional communication; foundations of disaster medicine; sanitary and hygienic maintenance of the medical salon; rules of sanitation and hygiene at work; the purpose of detergents and the rules for handling them; Traffic Laws; the basics of traffic safety; rules for the technical operation of vehicles (related to drivers); the main performance indicators of cars, ways and means of increasing labor productivity; signs, causes and dangerous consequences of malfunctions arising during the operation of the vehicle, methods of their detection and elimination; the procedure for the maintenance of cars and trailers; rules for storing cars in garages and open parking lots; rules for the operation of batteries and car tires; the influence of weather conditions on the safety of driving a car; ways to prevent road traffic accidents; first aid techniques in case of accidents; rules for filling out primary documents for accounting for the operation of the car; rules for running in new cars and after major repairs; the procedure for emergency evacuation of passengers in case of road traffic accidents; fundamentals of labor legislation; internal labor regulations; rules for labor protection and fire safety.
Qualification requirements. Secondary (complete) general education. Specialized training in first aid programs. The right to drive a car of category "B", work experience as a car driver for at least 3 years.
V. Positions of other personnel of medical and labor workshops at medical organizations Instructor of industrial training for workers of mass professionsJob responsibilities. Teaches unskilled workers, students modern techniques for performing production operations, working on machines, machine tools, etc. Plans educational work and keeps track of the implementation of industrial training programs, the results of the work of trainees and their progress. Carries out control over the provision of safe working conditions for trainees, over equipping their workplaces with everything necessary to perform the work. Conducts introductory and current briefings for all newcomers. It summarizes the best experience of work in the relevant profession, transfers it to trainees in order to improve their qualifications, introduce rational techniques and methods of work. Controls the correctness of the trainees' performance of technological operations, the quality and productivity of their labor. Conducts additional instructions for trainees to eliminate the identified deficiencies.
Should know: laws and other regulatory legal acts of the Russian Federation concerning the training of workers in mass professions; the basics of the treatment and diagnostic process, disease prevention; learning programs and the tariff and qualification characteristics of the respective occupations of the workers; principles and methods of industrial training; technological production process; arrangement and interaction of units and mechanisms of serviced machines and machine tools; rules for the technical operation of equipment; range of manufactured products; requirements for the quality of raw materials, semi-finished products and finished products; advanced methods of organizing, planning work and performing work practices and operations in the relevant professions; methods of monitoring and analyzing the work of trainees; the procedure for accounting for the implementation of industrial training programs, the results of the work of trainees and their progress; fundamentals of labor legislation; internal labor regulations; rules for labor protection and fire safety.
Qualification requirements. Secondary vocational education according to the profile of the work performed without presenting requirements for work experience or secondary (complete) general education, special training and work experience in the profile of at least 3 years.

The section "Qualification characteristics of positions of workers in the field of health care" of the "Unified qualification reference book of positions of managers, specialists and employees" (hereinafter referred to as the EKS), approved by Order of the Ministry of Health and Social Development of the Russian Federation of 23.07.2010 No. 541n(hereinafter referred to as Order No. 541n), qualification characteristics have been established for certain positions of specialists with higher medical education. Previously, there were qualification characteristics approved by Order dated 06.11.2009 No. 869... Many general provisions in the new regulatory legal act have remained unchanged.
The CEN and the qualifications themselves should contribute to:
- Correct selection and placement of personnel in the structure of medical personnel;
- medical personnel;
- Division of labor between managers, specialists;
- Ensuring the unity of approaches in the definition of these categories of medical personnel and those presented to them.
Moreover, qualification characteristics are used as regulatory documents, and also serve as the basis for the development of job descriptions of medical personnel, containing a specific list of job duties of medical personnel, taking into account the peculiarities of work.
Order No. 541n provides for qualification characteristics for:
- Leadership positions;
- Specialist positions (with higher medical, higher professional and secondary medical education);
- Positions of junior medical personnel;
- Positions of other personnel in medical and labor workshops at medical organizations.
Job titles for specialists with higher medical education are established in accordance with the Qualification Requirements for them, approved by the new Order of the Ministry of Health of Russia dated 08.10.2015 No. 707n which entered into force on 07.11.2015 (previously valid Order of the Ministry of Health and Social Development of Russia dated 07.07.2009 No. 415n). For example, the title of the position “general practitioner” corresponds to the medical specialty “therapy”.
The titles of the positions of medical specialists are established in accordance with the names of medical specialties, the nomenclature of which is approved in accordance with the established procedure ( Order of the Ministry of Health of Russia dated 07.10.2015 No. 700n(previously operated Order of the Ministry of Health and Social Development of the Russian Federation of 23.04.2009 No. 210n, invalidated on November 27, 2015), Order of the Ministry of Health and Social Development of the Russian Federation of April 16, 2008 No. 176n).
General qualification characteristic - "Doctor-specialist"
It should be borne in mind that Order No. 541n established a general qualification characteristic for the position of “Specialist Doctor”. It applies to those positions of medical specialists for which separate qualification characteristics are not provided for in this section of the TSA.
For specialists with higher professional (non-medical) education, hired for this position before October 1, 1999, the title of the position "Doctor-laboratory assistant" is retained.
A specialist with a medical education can receive the official title "Senior" if he is in charge of subordinate performers.
Order No. 541n also stipulates the situation when a specialist doctor is appointed to the position of a trainee doctor. Moreover, he:
- Has a break in work in the specialty for more than 5 years, or
- Changes the profile of the specialty.

At the same time, persons who do not meet the established qualification requirements, but have sufficient practical experience and perform the job duties assigned to them with high quality and in full, on the recommendation of the certification commission of the medical organization, as an exception, can be appointed to the relevant positions. The employer takes measures for further mentioned persons (Clause 6 of Order No. 541n).

1. To approve the qualification characteristics of health workers positions in accordance with the appendix to this order.
2. Department of Science and human resources The Ministry of Health of the Republic of Kazakhstan (Khamzina N.K.) to ensure, in the manner prescribed by law, state registration of this order in the Ministry of Justice of the Republic of Kazakhstan.
3. The Department of Administrative and Legal Work of the Ministry of Health of the Republic of Kazakhstan (Bismildin FB) shall ensure, in the manner prescribed by law, the official publication of this order in the media after its state registration.
4. Control over the execution of this order shall be entrusted to the Vice-Minister of Health of the Republic of Kazakhstan, Ye.A. Birtanov.
5. This order comes into effect upon the expiration of ten calendar days after the day of its first official publication.

Plans and organizes the effective work of the laboratory, ensures interaction with other departments of the organizations of the sanitary and epidemiological service. Draws up a work plan in conjunction with the state body for sanitary and epidemiological supervision. Conducts marketing research market of new technologies and research methods. Tests and implements new devices and equipment, methods of laboratory research and measurements. Organizes and participates in laboratory and instrumental studies of incoming samples. Plans and organizes the effective work of the vivarium, ensures interaction with other divisions of the organizations of the sanitary and epidemiological service. Analyzes the work of the vivarium and takes the necessary measures to improve its work. Takes part in laboratory and diagnostic research. Organizes the study of diseases, pathologies of animals kept in the vivarium. Provides effective financial and economic activities using new economic management methods. Conducts marketing research of the market for new technologies and research methods. Tests and introduces new devices and equipment into practice. Organizes work to ensure epizootic well-being in the colonies of laboratory animals. Ensures compliance with the bioethical rules for keeping animals and the anti-epidemic mode of manipulation to prevent the removal of infection from the laboratory. Analyzes the validity of the volume of use of experimental animals with the aim of possibly reducing experiments on the model of laboratory animals. Organizes safe working conditions for laboratory staff working with laboratory animals. Solves the issues of professional development of vivarium employees, their certification. Job responsibilities.

Conducts express analysis dosage forms, macro- and microscopic, commodity analysis of medicinal plant raw materials. Provides first aid to patients and victims in extreme situations. Provides storage and control of the shelf life of medicinal herbs and medical products. Participates in the acceptance, placement and packaging of goods using the necessary equipment. Conducts sanitary, educational and informational work on the use of medical devices. Keeps accounting and reporting documentation. Complies with medical ethics, internal labor regulations, fire safety and safety, sanitary and epidemiological regime.
Should know: The Constitution of the Republic of Kazakhstan, the Code of the Republic of Kazakhstan "On people's health and the health care system", the laws of the Republic of Kazakhstan "On permits and notifications", "On combating corruption", "On languages in the Republic of Kazakhstan", regulatory legal acts in the field of health care, pharmacy, the basics of pharmaceutical business, the structure of the drug supply management system, the modern nomenclature of drugs, herbs and medical products, the basic principles of the procurement and storage of medicinal plant raw materials, labor legislation, rules and regulations of labor protection, safety, industrial sanitation and fire safety.
Qualification requirements. Technical and vocational (secondary specialized, secondary vocational) (pharmaceutical) education.
